If you were not the driver at the time of the violation, complete the ‘Affidavit of Non-Liability’. By completing the ‘Affidavit of Non-Liability’, the citation issued to you will be cancelled after review and approval by the Court. The person named on the form will be cited and notified by mail. Mail the completed ‘Affidavit of Non-Liability’ on or before your appearance date to:

You will need the Notice Number and Pin Number printed on the front of the document sent to you by the agency to logon. You may also contact the Capitola Police department at (831) 475-4242 for more information.

If you were not the driver of the vehicle at the time of the violation, do not submit payment; otherwise, your driving record could be negatively affected. Complete and submit the affidavit of non-liability on or before the appearance date on the front of the citation and include a copy of your driver’s license.
